5th Grade Balarat Trip – Medication Forms needed!

The 5th Grade Balarat Trip is THIS WEEK! We need to know about any medications that your students will need to take while on this trip. If we already have all of the necessary medications on file with the Nurse’s office, then you don’t need to do anything further.

Any medication that needs to be taken outside of typical school hours will need to be turned in with the appropriate paperwork ASAP.

If you have medications that you need to send with your student, please view the attached General Medication Order that you can take to your child’s doctor. Keep in mind these need to be returned to the school as soon as possible with both your signature and your child’s medical provider’s signature. Should you have questions, feel free to contact our school Nurse by phone or email.

If you know your student will need to take medications that aren’t currently on file at school while on the Balarat Trip, you can print the attached forms out and bring directly to your doctor.
